Shoulder Pain? Start with an Ultrasound

“I think I hurt my rotator cuff.” We hear it almost every week. Sometimes that’s exactly right. But often the rotator cuff is only part of the story, and guessing at the cause is how shoulder pain ends up lingering for months.

The most direct way to find out what’s going on is a musculoskeletal (MSK) ultrasound, done right in our office during your visit.

If pain or injury is affecting your life, don’t wait for it to get worse. Our team at Virginia Orthopedics and Sports Medicine in Fairfax, VA is here to help you get back on the court. Call us at (703) 646-1270 to schedule an evaluation.

Stephanie Singh, DO, is a board-certified orthopedic and sports medicine physician at Virginia Orthopedics and Sports Medicine.

WHY THE ROTATOR CUFF ISN'T ALWAYS THE CULPRIT

Shoulder pain can come from many places, and the cuff is often reacting to a problem somewhere else in the joint:

• AC joint arthritis, which can crowd the space the cuff tendons pass through
• Glenohumeral joint wear or arthritis, which changes how the shoulder moves
• Labral or biceps tendon problems, which can mimic cuff pain
• Bursitis and impingement, which inflame tendons over time

Two people can describe the exact same pain and need completely different treatment. Without an accurate diagnosis, you risk treating the symptom while the real driver keeps causing damage.

WHAT AN ULTRASOUND SHOWS THAT A PHYSICAL EXAM CAN’T

A physical exam is essential, but it can’t see inside the shoulder. An MSK ultrasound can:

• Show tendons, bursae, and joint surfaces in real time, so we can identify tears, tendinopathy, fluid, and inflammation
• Watch the shoulder move. Dynamic imaging lets us see impingement and tendon behavior as you lift your arm, something still images often miss
• Distinguish cuff problems from joint problems in the same visit
• Give answers on the spot, so we can talk through your results and plan before you leave
• Involve no radiation, and no separate trip to an imaging center

WHY IT MATTERS FOR YOUR TREATMENT

Everything that follows depends on knowing the true source of your pain: which exercises to do, which activities to modify, and whether an injection is appropriate. Ultrasound also lets us guide any injection precisely to the structure that needs it, rather than treating the general area.

INJECTION THERAPY: A TARGETED NEXT STEP

When ultrasound shows an irritated tendon or a worn joint, injection therapy can help relieve symptoms and support long-term joint health alongside a strengthening program.

Platelet-rich plasma (PRP) uses a concentrated sample of your own blood, rich in platelets and growth factors, to support your body’s natural healing response. It’s commonly used for tendon problems, including rotator cuff tendinopathy and some partial tears, as well as wear-and-tear joint pain.

Prolotherapy uses a dextrose solution to stimulate healing in irritated ligaments, tendons, and joint structures, and is often considered when chronic overload or instability contributes to pain. It usually takes a series of treatments, generally two to five injections over time, depending on your response.

Neither is a quick fix, and results vary. The goal is to improve symptoms and support the tissue, so you’re not just masking pain.

DON’T WAIT TO GET IT EVALUATED

Schedule a shoulder ultrasound if you have:

• Shoulder pain lasting more than a couple of weeks
• Pain that wakes you at night
• Weakness lifting your arm or reaching overhead
• Pain that keeps coming back after rest

Joint health importance and improvement should be part of our daily regiment

Ways to improve bone and joint health include a diet rich in fresh fruits and vegetables. Leafy greens are especially rich in calcium and other minerals that can help both your joints and bones. If you exercise, be sure to choose low impact exercises that do not place too much strain on your joints. Many people choose walking, cycling or swimming to stay in shape. Yoga and gentle stretching can help keep your joints more flexible, too. Lifting small weights can also improve your bone density.

Exercise also improves mood, reduces stress and boosts your immune system. The best time to fight osteoporosis is before it begins. Bone and joint health are a part of your overall wellness. You should also talk to your doctor, as certain medications (even over the counter ones), can leech calcium from your bones.
